We are dedicated, purpose driven, professional and deeply committed to our “care to the core” approach in all aspects of our service delivery and seek a dedicated and values driven Care Leader / Case Manager to join us.
This role is a permanent, full-time opportunity and the main location of the placements will be in Ipswich and Brisbane areas.
About the Role
Working across residential out-of-home care services located across the South West Ipswich and Brisbane. Your role will see you lead a small team of passionate Care Practitioners and a Senior Care Practitioner who provide 24/7 care, mentoring and support to a group of young people.
Reporting to the Area Manager, your passion for supporting young people thrive and your youth worker teams develop will be utilised as you lead and contribute to innovative outcomes driven care plans and facilitate an environment of positive engagement and outcomes.
Some of the tasks will include:
- Ensuring the young people in our care are supported, nurtured and have a safe place to thrive in whilst in our care.
- Inducting, onboarding, training and mentoring youth work staff to achieve the goals of their roles ensuring they are leveraging all organisation systems and tools to be effective and efficient in their delivery of care.
- Providing supervision and development plans for youth work staff for continuous improvement and support of their professional development.
- Engaging and developing robust relationships with key stakeholders delivered with a customer service approach.
- Fulfilling reporting requirements in relation to the young people and workers, including case management, safety plans, positive behaviour plans and other legislative requirements.
- Develop rosters being mindful of award compliance, service agreement requirements, cost-effectiveness and ensuring our young people are always supported via shift teams as per approved models.
- Support youth workers via after hours on-call rotation.
About You
Passion for supporting vulnerable young people is critical. You’re known for your above and beyond approach and commitment to delivering quality care and you’re equally passionate about inspiring, supporting, mentoring and guiding teams of youth workers to deliver the best care outcomes possible. Being values driven and having a desire to work with a broader team who truly live these values will be something that you are seeking, because that is who we are and our why.
Naturally, you have experience in the field of young people residential care and are a Diploma level holder. Further you will hold all the required legislative requirements and are very conversant with the Child Protection Act 1999 and knowledge of the impacts of trauma on young people.
This includes:
- A current QLD Working with Children Check (Paid Blue Card)
- A valid driver’s license with no restrictions
- A national police check
- A First Aid and CPR Certificate
- Completion of Hope and Healing training
